To create this card I have used the following: 
White card blank
White cardstock
Yellow cardstock
Tonic scallop rectangle dies
Image from the Dog Breed Stamps Colouring Book
by Radiant Artistry Designs coloured with Copics
Crafters Companion stamped sentiment
White gel pen
